What should you ask your prescriber when choosing between them?
Lexapro vs Prozac is a question of fit more than strength. Both are SSRIs and both work better than placebo for depression. They differ in how long they stay in the body, in approved uses and in how individual people respond.
How are Lexapro and Prozac different?
Lexapro (escitalopram) is approved for major depressive disorder and generalized anxiety disorder. Prozac (fluoxetine) is approved for major depressive disorder, OCD, panic disorder and bulimia. The biggest practical difference is half-life: Prozac and its active breakdown product stay in the body for weeks. For you, that means a missed Prozac dose is barely felt and stopping it is often gentler, while a dose change takes weeks to show fully. Lexapro clears in days, so missed doses and stopping are felt sooner. The doses in the table are not comparable milligram for milligram: 10 mg of Lexapro and 20 mg of Prozac are each a standard starting dose for depression.
Feature Lexapro Prozac Generic name escitalopram fluoxetine Approved uses depression, generalized anxiety depression, OCD, panic, bulimia Usual starting dose for depression 10 mg 20 mg Maximum dose 20 mg 80 mg Half-life 27-32 hours 4-6 days with daily use; its active form 4-16 days Youngest approved age 12 for depression, 7 for anxiety 8 for depression, 7 for OCD
Is Lexapro or Prozac more effective?
On average Lexapro came out a little ahead, but for any one person the difference is small. In head-to-head trials in a 2018 analysis of 522 trials in adults with depression, escitalopram was among the more effective drugs and fluoxetine among the less effective, while both were among the better tolerated. All 21 antidepressants in the analysis beat placebo. The average gap between drugs is small next to how differently individual people respond, and the certainty of the evidence was rated moderate to very low.
Which has fewer side effects, Lexapro or Prozac?
Neither is milder across the board. Comparing the depression trials in each label, nausea affected 15% on Lexapro against 7% on placebo, and 21% on Prozac against 9%. Insomnia affected 9% on Lexapro against 4%, and 16% on Prozac against 9%; anxiety affected 12% on Prozac against 7%. The labels' figures come from different trials, so treat them as a rough guide rather than a head-to-head.
Sexual side effects are a big reason people switch. Lexapro's label lists delayed ejaculation in 9% of men against under 1% on placebo, plus lower libido and trouble reaching orgasm. Prozac's lists abnormal ejaculation, impotence and lower libido. In Lexapro's anxiety trials, side effects ran higher than in its depression trials, with nausea at 18% against 8%. Sleepiness in the depression trials affected 13% on Prozac against 6% on placebo, and 6% on Lexapro against 2%.
Both labels carry a boxed warning that suicidal thoughts can rise in children, teenagers and young adults aged 24 and younger, most of all in the first months and after a dose change.
What 297 people on Reddit said about Lexapro and Prozac
We read 100 recent threads where people compared Lexapro and Prozac on r/lexapro, r/prozac and other subs, comments included, and 297 people described their own experience. These are people's own accounts, not how often each effect happens, and people with a strong preference are more likely to post, so treat this as a range of experiences, not a ranking.
People who had tried both were split about evenly on which worked better: of the 66 who said, 31 preferred Lexapro and 35 Prozac. Of the 257 who said what they took, 176 had taken both, 36 only Lexapro and 45 only Prozac, and each column counts the people who took that drug. One person can be in several rows:
What people noticed On Lexapro On Prozac Tiredness 48 22 Weight gain 40 8 Less anxiety 34 18 More anxious or agitated 14 42 Better mood 10 22
When people who had taken both explained why they switched, the most common reason was side effects (43 people), then the first one stopped working (12). One person who moved to Prozac wrote:
my energy is soooo much better on Prozac
Feeling more anxious or agitated came up on Prozac for 42 people. New or worse anxiety, agitation or insomnia in the first weeks is something to raise with your prescriber, not to sit out.
Is Lexapro or Prozac better for anxiety?
It depends on the kind of anxiety. Lexapro is approved for generalized anxiety disorder; Prozac is approved for panic disorder and OCD. Prozac isn't approved for generalized anxiety, which doesn't mean it is never used for it, but it does mean the label's evidence is for other conditions. Tell your prescriber whether your anxiety is mostly constant worry, panic attacks or obsessions and compulsions, since that steers which label fits.
How do doctors choose between Lexapro and Prozac? Clinical section: medical review pending
For this pair, three things usually decide it. The first is half-life: Prozac lingers for weeks, Lexapro clears in days. The second is approved use: Lexapro for generalized anxiety, Prozac for OCD, panic disorder and bulimia. The third is how you react: in people's accounts on Reddit, Prozac more often felt activating and Lexapro tiring, though in the trials sleepiness was reported more often on Prozac. The clinical view:
In large comparisons most antidepressants work about equally well on average. They differ more in side effects, interactions and how hard they are to stop. Needing a second choice does not mean the first was a mistake.
Prescribers look at your main symptoms, other conditions, other medications and past response, yours or a close relative's. Some antidepressants are more activating and others more sedating, which can be matched to problems with sleep or energy.
If the first one doesn't help enough, putting up with it and stopping are not the only options; which ones fit depends on you, and it is a conversation with your prescriber.

What should you ask your prescriber when choosing between them?
A clear picture of your symptoms and history makes the choice faster. If you have tried one already, details of how it went help most. It helps to ask or tell them:
which symptom matters most: low mood, worry, panic or OCD how you did on either drug before, or on any other SSRI whether tiredness, weight or feeling wired would be hardest to live with whether you often miss doses whether you're pregnant, planning a pregnancy or breastfeeding if switching, how the change will be planned

Frequently asked questions How long should you give Lexapro or Prozac before deciding? Several weeks at a steady dose for either. Prozac's long half-life means a dose change takes longer to settle, so allow a little more time after any change and keep notes for your prescriber.
Can you take Lexapro and Prozac together? They are not usually combined, because two SSRIs together raise the risk of serotonin syndrome. Your prescriber plans any switch.
Which causes more weight gain, Lexapro or Prozac? In our Reddit count, weight gain came up far more often on Lexapro, 40 people against 8. That is what people describe, not a measured rate. Prozac's label has its own warning about weight loss: in depression trials, 11% on Prozac had a smaller appetite against 2% on placebo. Raise any steady change with your prescriber.
Is it easier to stop Prozac or Lexapro? Prozac's long half-life makes it taper itself as levels fall over weeks, so withdrawal symptoms are often milder or come later. Lexapro clears within days, so a plan for a gradual reduction matters more. Either way, plan stopping with your prescriber.
