Key takeaways Lexapro is FDA-approved for generalized anxiety disorder in adults and children from 7; like all antidepressants, it carries a warning about suicidal thoughts in people 24 and younger. In three 8-week trials it eased anxiety more than placebo, and a 2019 review of 89 trials found it effective with relatively good acceptability. In 100 r/lexapro threads, most people who said how their anxiety changed said it got better, some only after a rough start. Anxiety can rise in the first days; new panic attacks or feeling very agitated need a call to your prescriber, not just waiting it out.

Lexapro (escitalopram) is used for anxiety and is FDA-approved for generalized anxiety disorder (GAD). In the UK it is also used for panic disorder, social anxiety and OCD. The benefit builds over several weeks, and some people feel more anxious before they feel better.
How anxiety changed for 155 people on r/lexapro
We read 100 recent threads about anxiety on r/lexapro, comments included, and 456 people described their own experience. These are people's own accounts, not trial scales, and people with strong experiences are more likely to post, so treat this as a range of experiences. Of the 456, 239 said what they took it for, mostly anxiety (216), then panic attacks (57) and OCD (21); one person could name more than one reason, so the results speak mainly to anxiety.
Of the 155 people who said how their anxiety changed, 99 said it got better, 23 partly better, and 14 that it got worse at first and then better.

My anxiety got so much worse before it got better.
Does Lexapro help with anxiety?
On average, yes. The label is based on three 8-week studies in adults aged 18 to 80, where Lexapro at 10 to 20 mg a day eased anxiety more than placebo in all three. A further 8-week study supported its use in children and teenagers from age 7.
A 2019 review in The Lancet pooled 89 trials with 25,441 people with GAD. Escitalopram was more effective than placebo, by about 2.5 points on the Hamilton anxiety scale, and had relatively good acceptability, meaning people did not drop out of trials much more often than on placebo. Duloxetine, pregabalin and venlafaxine were in the same group. These are averages across groups of people, so they cannot say how well it will work for you.
How long does it take Lexapro to work for anxiety?
Several weeks, the NHS says, before anxiety symptoms start to improve. In the label's GAD trials the benefit was measured at week 8, so it was clear by then; that does not mean it takes 8 weeks to start. What most people notice first is side effects, not relief: Lexapro acts from the first doses, but the brain's response builds more slowly. The NHS adds that most side effects ease after a couple of weeks. Our Reddit count did not record when anxiety first eased, so we can't add a typical number from there.
How the first weeks usually go, in people's own words, is covered in how long Lexapro takes to work .
Why can anxiety get worse when you start Lexapro? Clinical section: medical review pending
A short rise in anxiety is a known early side effect. The NHS lists feeling anxious or restless among the common side effects of escitalopram. The label starts all adults at 10 mg once a day, for anxiety and depression alike, and allows 20 mg after at least a week. A prescriber may start lower if you are sensitive to side effects; the NHS notes that a low starting dose reduces the chance of them. The clinical view:
Antidepressants are a first-line treatment for most anxiety disorders, and their benefit for anxiety builds over several weeks.
People with anxiety can be more sensitive to early side effects, including a brief rise in anxiety. That is why treatment often starts at a low dose and goes up slowly.
For many people the rise eases within a couple of weeks. Some changes are not something to sit out. The FDA label asks you to call your prescriber right away about new or worse anxiety, panic attacks, feeling very agitated or restless, or trouble sleeping, especially early on or after a dose change. It carries a boxed warning that suicidal thoughts can rise in children, teenagers and young adults aged 24 and younger. Since Lexapro is approved for anxiety from age 7, parents should watch for these changes too.
Other early effects are covered in Lexapro side effects .
What dose of Lexapro is used for anxiety?
The label gives the same range for GAD as for depression: start at 10 mg once a day, with a maximum of 20 mg. Your prescriber may start lower if you are sensitive to side effects. The right dose is the lowest one that controls your anxiety with side effects you can live with.
What should you tell your prescriber about anxiety on Lexapro?
Tell your prescriber how your anxiety has changed since you started, including any rise in the first days. That helps them decide whether to wait, adjust the dose or try something else. It helps to tell them:
When you started Lexapro, your dose and any dose change. Whether anxiety rose at first, and whether it has eased since. Which symptoms have changed: worry, panic attacks, physical tension, sleep. Any side effects that make it hard to keep going. Whether you’re in therapy or have used other medications for anxiety.
If anxiety turns into thoughts of harming yourself, get help right away: call your prescriber, or in the US call or text 988, or find a crisis line near you .
Download on the App Store Anxiety on Lexapro often gets better in steps, sometimes after a rough start. The withClaro app lets you log mood, sleep, energy and side effects each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions Is Lexapro good for panic attacks? It is used for panic disorder in the UK, though in the US its FDA approval is for GAD and depression. Ask your prescriber whether it fits your type of anxiety.
How long does the early rise in anxiety last on Lexapro? For many people it eases within the first couple of weeks, in line with the NHS note that most side effects settle as the body gets used to the medicine. If it keeps building after that, or comes with panic attacks you didn't have before, tell your prescriber.
Is 5 mg of Lexapro enough for anxiety? The trials behind the label tested 10 to 20 mg, so there is no trial evidence on whether 5 mg is enough. Some prescribers use it as a starting dose so that side effects are milder, then go up to 10 mg. If you have stayed on 5 mg and your anxiety hasn't eased, raise it with your prescriber.
Should I take Lexapro in the morning or at night for anxiety? Either works. Pick the time that suits your sleep and keep it steady. If a morning dose leaves you jittery, ask about an evening one.
