Key takeaways Prozac is FDA-approved for panic disorder and OCD; for generalized anxiety it is prescribed off label. In two 12-week panic trials, more people on Prozac were free of panic attacks than on placebo: 42% against 28%, and 62% against 44%. In 100 r/prozac threads, most people who said how their anxiety changed said it got better, some only after a rough start. Anxiety can rise in the first weeks; extreme worry, agitation or new panic attacks are a reason to call your prescriber right away, especially for people 24 and younger.

What 151 people on r/prozac said about anxiety Does Prozac help with anxiety and panic attacks? Does Prozac work for OCD? Why can anxiety get worse when you start Prozac? What should you tell your prescriber about anxiety on Prozac?
Prozac (fluoxetine) is used for anxiety and is FDA-approved for panic disorder and obsessive compulsive disorder (OCD). For generalized anxiety it is prescribed off label, and a 2019 review found it effective there too, though the trials were small. In the UK the NHS lists depression, OCD and bulimia, so there its use for anxiety and panic is also off label. The benefit builds over weeks, and some people feel more anxious first.
What 151 people on r/prozac said about anxiety
We read 100 recent threads about anxiety on r/prozac, comments included, and 484 people described their own experience. These are people's own accounts, not trial scales, and people with strong experiences post more, so treat this as a range of experiences.
Of 151 who said how their anxiety changed, 81 said it got better, 19 partly better, and 28 felt worse at first and then better. In the table, "no change yet" means it was still early, while "didn't help" means they gave it time without improvement:
How their anxiety changed Of 151 people Better 81 Partly better 19 Worse at first, then better 28 No change yet 12 Didn’t help 5 Worse 6
Of the 484, 257 said what they were treating: 197 named anxiety, 72 OCD and 48 panic attacks. Some named more than one, so these add up to more than 257. One person on r/prozac wrote:
I was on Prozac for 12 years and for me it really helped my anxiety and panic attacks.
Another described a harder start:
i had horrible anxiety attacks and waking up with panic and the whole day was horrible from week 2 to week 6 but then it got better no more anxiety attacks and panic attacks
Does Prozac help with anxiety and panic attacks?
Yes for panic disorder and OCD, where it is approved and backed by trials; for generalized anxiety it is used off label and the evidence is weaker. In two 12-week studies, people started on 10 mg and moved to 20 to 60 mg. In the first, 42% on Prozac were free of panic attacks at the end, against 28% on placebo; in the second, 62% against 44%.
For generalized anxiety, a 2019 review in The Lancet pooled 89 trials with 25,441 people. The drugs with the clearest results there were duloxetine, pregabalin, venlafaxine and escitalopram. Fluoxetine was in a second group that also worked, but its evidence rested on small trials, so the result is less certain.
Does Prozac work for OCD?
Yes, OCD is one of its approved uses. Two 13-week adult studies and a 13-week study in children and teenagers supported the approval. OCD often needs higher doses and more time: the label gives a range of 20 to 60 mg and says the full effect may take 5 weeks or longer.
Why can anxiety get worse when you start Prozac? Medically reviewed by Dr. Jason Tan on October 27, 2026, this section only.
Prozac acts on serotonin from the first doses, while its calming effect on anxiety takes weeks to build, and the label lists anxiety and insomnia as a separate warning. In its OCD trials, 14% reported anxiety against 7% on placebo, and 28% insomnia against 22%. In the panic disorder trials, 6% felt anxious and 8% nervous, against 2% and 6% on placebo. For panic disorder, the label starts at 10 mg for a week, then 20 mg. The clinical view:
Antidepressants are a first-line medication for most anxiety disorders, alongside talking therapy, and their benefit for anxiety builds over several weeks.
People with anxiety can be more sensitive to early side effects, including a brief rise in anxiety. That is why treatment often starts at a low dose and goes up slowly. On Reddit, people who felt worse first often said it settled within a few weeks. Some rises are not something to wait out. MedlinePlus says to call your doctor right away for extreme worry, agitation, panic attacks or trouble sleeping. It adds that the risk of suicidal thoughts is higher in people 24 and younger, especially early on or after a dose increase.
What should you tell your prescriber about anxiety on Prozac?
Tell your prescriber how your anxiety has changed since you started, including any rise in the first weeks. It helps to tell them:
When you started Prozac, your dose and any dose change. Whether anxiety rose at first, and whether it has eased since. Which symptoms have changed: worry, panic attacks, intrusive thoughts, sleep. Any side effects that make it hard to keep going. Whether you’re in therapy or have used other medications for anxiety.
If anxiety brings thoughts of harming yourself, tell your prescriber right away. If you're having thoughts of harming yourself, contact emergency services or a crisis line straight away. In the US, call or text 988. In the UK, call Samaritans on 116 123. Elsewhere, find a crisis line near you .
Download on the App Store Anxiety on Prozac often eases in steps, sometimes after a rough start. The withClaro app lets you log mood, sleep, energy and side effects each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions Can you take Prozac only when you feel anxious? No. It works by building up over weeks of daily use, so a single dose on a bad day does little. If you need something for sudden spikes, ask your prescriber what fits alongside it.
How long does the early rise in anxiety last? Of the 28 people on r/prozac who felt worse first and then better, 18 said how long they had been taking it when they posted: typically about 5 weeks. Extreme worry, agitation or new panic attacks are not something to wait out: call your prescriber right away.
Is Prozac or Zoloft better for anxiety? Both are SSRIs used for anxiety. Zoloft is approved for panic, social anxiety and PTSD; Prozac for panic and OCD. Ask your prescriber which fits you.
Can Prozac make OCD worse at first? Some people report a rough start. Tell your prescriber if intrusive thoughts get worse after starting or a dose change.
