Key takeaways The FDA label says the full effect for depression may take 4 weeks or longer, and for OCD 5 weeks or longer. Fluoxetine and its active form build up over 4 to 5 weeks, which is part of why it is slow to settle. Of 30 people on r/prozac who said when it started working, the typical answer was about 4 weeks, in line with the label. A rise in anxiety in the first weeks is common; thoughts of self-harm are a reason to call your prescriber the same day.

When did Prozac start working for people on r/prozac? Why does Prozac take so long to work? What are the signs Prozac is working? Why can anxiety get worse when you start Prozac? What should you tell your prescriber in the first weeks on Prozac?
How long does it take Prozac to work? For depression, Prozac (fluoxetine) can take 4 weeks or longer to reach its full effect, the FDA label says, and 5 weeks or longer for OCD. The NHS says it can take a few weeks before symptoms start to improve. Fluoxetine acts from the first dose, but the brain takes weeks to respond, so side effects often come first and the early weeks can feel worse.
When did Prozac start working for people on r/prozac?
We read 100 recent threads about the first weeks on r/prozac, comments included, and 274 people described their own experience. Many threads are written in the hard early weeks, which is why so many people said it had not worked yet, so treat this as a range of experiences. Of the 274, 224 said how it was going:
How it was going Of 224 people It worked 72 Partly working 53 Not yet 94 It didn’t work 5
Of the 72 who said it worked, 30 said when it started: the typical answer was about 4 weeks, and half said between 2 and 5 weeks. That is a small group, so take it as a rough guide. Some felt it early; one person wrote:
by week 2 I started noticing my mood getting a lot better
For others it took longer:
My week 6 was when I felt better.

Two things add up. Like other antidepressants, fluoxetine acts from the first dose, but the brain takes weeks to adjust to it. On top of that, fluoxetine builds up slowly. With daily use, its half-life is 4 to 6 days, and the half-life of its active breakdown product, norfluoxetine, is 4 to 16 days, so steady levels take 4 to 5 weeks. The same long half-lives mean the medicine stays in the body for weeks after the last dose, which sets Prozac apart from other SSRIs, and a dose change also takes several weeks to show fully. The clinical view:
Antidepressants often start to help within one to two weeks, but the full effect takes several weeks, and side effects often show up first.
With some antidepressants, a short rise in anxiety or restlessness at the start is common and usually settles. If it is hard to manage, your prescriber may lower the starting dose or change the time of day.
Early signs of progress are often small, and other people may notice them first. Getting out of bed gets easier, you spend less time on the same worries, and old interests come back.
Talk to your prescriber about the dose or a different medication if:
nothing has improved after about four weeks at a steady dose side effects are hard to live with
New or worse thoughts of suicide or self-harm in the first weeks, especially in people under 25, need a call to your prescriber the same day. If you feel you might act on them, call 988 or 911 now. The FDA boxed warning says this risk is higher in children, teenagers and young adults up to age 24. If you're having thoughts of harming yourself, contact emergency services or a crisis line straight away. In the US, call or text 988. In the UK, call Samaritans on 116 123. Elsewhere, find a crisis line near you .
For OCD the label puts the full effect at 5 weeks or longer, and OCD often needs a higher dose than depression.
What are the signs Prozac is working?
The first signs are often small and easy to miss from day to day: getting out of bed is a bit easier, worries take up less of the day, and things you used to enjoy start to interest you again. People close to you may notice before you do. If several weeks at a steady dose bring no change, the label says a dose increase can be considered, and that is a decision for your prescriber.

Why can anxiety get worse when you start Prozac?
Fluoxetine changes serotonin signaling from the first doses, before the brain has adjusted, and that can show up as nervousness or poor sleep. On r/prozac it was the early side effect named most often: 63 of the 274 said their anxiety got worse at first. In Prozac's depression trials, 14% felt nervous and 12% anxious, against 9% and 7% on placebo, counted over the whole trials rather than the first weeks alone.
What should you tell your prescriber in the first weeks on Prozac?
Tell your prescriber how you have felt since starting, including side effects and any rise in anxiety, and ask whether that is expected at this point, how long it may last and when you will review it together. It helps to tell them:
When you started Prozac and your dose. What has changed, even small things such as sleep or energy. Any rise in anxiety, restlessness or low mood. Side effects that make it hard to keep going. Whether you have missed doses.
Download on the App Store The first changes on Prozac are small and easy to miss from one day to the next. The withClaro app lets you log mood, sleep, energy and side effects each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions Can Prozac work in the first week? Some people notice changes in sleep or energy early, but the effect on mood usually takes several weeks.
Is week 3 on Prozac the hardest? For some people the early weeks bring more anxiety or poor sleep before mood lifts. Tell your prescriber if it is hard to bear.
How long does Prozac take to work for anxiety? Usually several weeks, like for depression. If anxiety rises at the start, tell your prescriber how strong it is and when it comes.
How long does Prozac take to work for OCD? The label says 5 weeks or longer, longer than for depression. Agree with your prescriber how long to give each dose.
What if Prozac hasn't worked after 6 weeks? Talk to your prescriber, and bring what has changed and what has not, your dose and how long you have been on it, and which side effects are hard to live with. Waiting it out and stopping are not the only two options, and stopping on your own can bring withdrawal symptoms.
