Key takeaways Zoloft and Prozac are both SSRIs approved for depression, OCD and panic disorder. Prozac stays in the body for weeks; Zoloft has a half-life of about a day. Missing a dose or stopping feels different: Zoloft leaves the body within days, Prozac over weeks. On Reddit, people more often linked Zoloft with weight gain and feeling flat, and Prozac with feeling more anxious; most threads came from r/prozac, so preference counts lean that way.

Zoloft vs Prozac is a question of fit more than strength. Both are SSRIs, both are approved for depression, OCD and panic disorder, and both are among the better tolerated antidepressants in large analyses. They differ in how long they stay in the body, in other approved uses and in how individual people respond.
How are Zoloft and Prozac different?
Zoloft (sertraline) is approved for major depressive disorder, OCD, panic disorder, PTSD, social anxiety disorder and PMDD. Prozac (fluoxetine) is approved for major depressive disorder, OCD, panic disorder and bulimia, and together with olanzapine for depression in bipolar I disorder and for depression that hasn't responded to two other antidepressants. Prozac and its active breakdown product stay in the body for weeks.
Feature Zoloft Prozac Generic name sertraline fluoxetine Approved uses depression, OCD, panic, PTSD, social anxiety, PMDD depression, OCD, panic, bulimia; with olanzapine, bipolar depression and treatment-resistant depression Usual starting dose for depression 50 mg 20 mg Maximum dose 200 mg 80 mg Half-life about 26 hours 4-6 days with daily use; its active form 4-16 days Common side effects in trials nausea, diarrhea, insomnia nausea, insomnia, nervousness, anxiety A missed dose or stopping levels fall within days, so missed doses and stopping are felt sooner levels fall over weeks, so a missed dose matters less and withdrawal tends to start later
The doses are not comparable milligram for milligram. 50 mg of Zoloft and 20 mg of Prozac are each a standard starting dose for depression.
Is Zoloft or Prozac more effective?
Neither is reliably stronger for a given person, and the gap in head-to-head trials is small and uncertain. A 2018 analysis of 522 trials in adults with depression found all 21 antidepressants it studied beat placebo. In head-to-head trials, fluoxetine was among the less effective drugs, while both sertraline and fluoxetine were among the better tolerated. The certainty of that evidence was rated moderate to very low. On Reddit, more people who had tried both preferred Prozac. The two don't contradict each other: trials measure the average improvement on a scale, while people pick the drug that is easier to live with, and the threads lean toward r/prozac.
Which has fewer side effects, Zoloft or Prozac?
Neither is milder across the board; Zoloft leans toward stomach effects and Prozac toward nervousness and anxiety. The labels warn that figures from one drug's trials can't be compared directly with another's; Zoloft's pool covers six conditions and Prozac's four. With that caveat, in Zoloft's trials nausea affected 26% of people, diarrhea 20% and insomnia 20%, against 12%, 10% and 13% on placebo. In Prozac's, nausea affected 22% against 9%, insomnia 19% against 10%, and anxiety 12% against 6%.
Zoloft vs Prozac: which do people on Reddit prefer?
We read 100 recent threads comparing the two, comments included, and 242 people described their own experience. These are people's own accounts, and about two thirds of the threads came from r/prozac and r/fluoxetine, so people happy on Prozac are overrepresented, and people with a strong preference post more.
Of the 242, 138 had taken both, 46 only Zoloft and 51 only Prozac; 7 didn't say. Each column counts the people who took that drug. Of the 138 who had taken both, 52 said which worked better: 34 preferred Prozac, 14 Zoloft and 4 neither. One person can be in several rows:
What people noticed On Zoloft On Prozac Weight gain 29 5 Less anxiety 26 11 More anxiety 10 21 Feeling flat or numb 26 9 Tiredness 20 17 Stomach upset 11 13
Of the 138 who had taken both, 60 said why they switched: side effects for 39, the first one stopping working for 10, never helping for 7 and other reasons for 4. One person who moved to Prozac wrote:
Prozac has put in the work and I’ve had fewer side effects
If you are on Zoloft and the side effects people describe here are the ones bothering you, don't switch on your own. Bring your prescriber which side effect it is, since when, and how much it gets in your way; a switch is planned with them.
How do doctors choose between Zoloft and Prozac? Clinical section: medical review pending
For this pair, three things usually decide it. Approved uses: Zoloft covers PTSD and social anxiety, Prozac bulimia. Half-life: Prozac's long tail suits people who sometimes miss doses, while Zoloft's shorter one makes dose changes quicker to judge. And how you react, which you only learn by trying. The clinical view:
In large comparisons most antidepressants work about equally well on average. They differ more in side effects, interactions and how hard they are to stop. Needing a second choice does not mean the first was a mistake.
Prescribers look at your main symptoms, other conditions, other medications and past response, yours or a close relative's. Some antidepressants are more activating and others more sedating, which can be matched to problems with sleep or energy.
Choosing and switching are both planned with your prescriber. Our guide to switching antidepressants explains the usual approaches.
Is Zoloft or Prozac better for anxiety?
Both are approved for panic disorder and OCD. Zoloft is also approved for PTSD and social anxiety disorder; neither is approved for generalized anxiety disorder. In our Reddit count, 26 people said anxiety eased on Zoloft against 11 on Prozac, and 21 said it rose on Prozac against 10 on Zoloft; in Prozac's trials, anxiety was a side effect for 12% against 6% on placebo. So for panic disorder or OCD, both are on the label; for PTSD or social anxiety, only Zoloft is; for generalized anxiety, neither is, and your prescriber may still use one of them.
What should you ask your prescriber when choosing between them?
A clear picture of your symptoms and history makes the choice faster. If you have tried one already, details of how it went help most. It helps to ask or tell them:
which symptom matters most: low mood, panic, OCD, trauma or social anxiety how you did on either drug before, or on any other SSRI whether weight, feeling flat or feeling wired would be hardest to live with whether your stomach is sensitive whether you're pregnant, planning a pregnancy or breastfeeding, since that changes the conversation if switching, how the change will be planned
Some changes can't wait for a planned visit. Both labels carry a boxed warning that suicidal thoughts can rise in people 24 and younger, especially early on and after a dose change; a sharp drop in mood or thoughts of self-harm need help right away. Call your prescriber, or in the US call or text 988; in the UK, call NHS 111 or 999; elsewhere, find a crisis line near you .

Frequently asked questions How do you switch from Zoloft to Prozac? Your prescriber plans it. Because Prozac stays in the body for weeks, switches involving it need care with timing; some prescribers lower one while starting the other, others stop one first. Ask how long the change will take and what to watch for.
Can you take Zoloft and Prozac together? They are not usually combined, because two SSRIs together raise the risk of serotonin syndrome. Your prescriber plans any switch.
Which causes more weight gain, Zoloft or Prozac? In our Reddit count, weight gain came up far more often on Zoloft. Raise any steady change with your prescriber.
Why switch from Zoloft to Prozac? In our count, side effects were the most common reason. Discuss the reasons with your prescriber before you switch.
