Key takeaways Lexapro and Zoloft are both SSRIs, and in large analyses they were among the better tolerated antidepressants. For anxiety the labels split: Lexapro is approved for generalized anxiety disorder, Zoloft for panic disorder, PTSD and social anxiety, and Zoloft also for OCD. Of people on Reddit who took both and said which worked better, 36 preferred Lexapro and 30 Zoloft, an almost even split. On Reddit, people more often linked Zoloft with stomach upset and Lexapro with tiredness; the labels' trial figures come from different conditions and can't be compared directly.

What should you ask your prescriber when choosing between them?
Lexapro vs Zoloft is mostly a question of fit, not strength. Both are SSRIs and the two most prescribed antidepressants in the US. They differ in approved uses, in their most common side effects and in how individual people respond.
How are Lexapro and Zoloft different?
Lexapro (escitalopram) is approved for major depressive disorder and generalized anxiety disorder. Zoloft (sertraline) is approved for major depressive disorder, OCD, panic disorder, PTSD, social anxiety disorder and PMDD, but not for generalized anxiety. For someone with anxiety, that split is often the most practical difference. Both are taken once a day, and both have a half-life of about a day.
Feature Lexapro Zoloft Generic name escitalopram sertraline Approved uses depression, generalized anxiety depression, OCD, panic, PTSD, social anxiety, PMDD Usual starting dose 10 mg 50 mg (25 mg for panic, PTSD, social anxiety) Maximum dose 20 mg 200 mg Half-life 27-32 hours about 26 hours Most common side effect in trials nausea nausea and diarrhea
The doses are not comparable milligram for milligram, and neither are the maximums: 20 mg of Lexapro and 200 mg of Zoloft are each the top of their own scale. 10 mg of Lexapro and 50 mg of Zoloft are each a standard starting dose for depression.
Is Lexapro or Zoloft more effective?
On average the difference is small and not reliable; the practical differences are in side effects and approved uses. A 2018 analysis of 522 trials in adults with depression found all 21 antidepressants worked better than placebo. In head-to-head trials, escitalopram was among the more effective drugs and sertraline was not on that list, while both were among the better tolerated. Most of those comparisons had wide ranges, and the certainty of the evidence was rated moderate to very low, so the true difference could be larger, smaller or none at all. Either way, judge it by how you feel after several weeks.
Which has fewer side effects, Lexapro or Zoloft?
Neither one is milder across the board; their side effects differ in kind. Zoloft's label puts stomach effects first: nausea, diarrhea and indigestion, along with tremor and sweating. Lexapro's puts nausea, insomnia, sweating, sleepiness and fatigue among its most common. In Zoloft's trials, pooled across six conditions, nausea affected 26% of people and diarrhea 20%, against 12% and 10% on placebo. In Lexapro's depression trials, nausea affected 15% against 7% on placebo, and diarrhea 8% against 5%, while in its anxiety trials nausea reached 18% against 8%. Both labels say figures from different trials can't be compared directly, so treat this as a rough picture. Both commonly cause sleep changes, sweating and sexual side effects. If your stomach is sensitive, that is worth raising when the two are being weighed.
What 294 people on Reddit said about Lexapro and Zoloft
We read 100 recent threads where people compared Lexapro and Zoloft on r/lexapro, r/zoloft and other subs, comments included, and 294 people described their own experience. These are people's own accounts, not trial measurements, and people for whom a drug didn't work post more, so treat this as a range of experiences.
Of the 294, 180 had taken both, 48 only Lexapro and 47 only Zoloft; 19 didn't say. Each column counts the people who took that drug: those who took both plus those who took only it. Of the 180 who had taken both, 69 said which worked better: 36 preferred Lexapro, 30 Zoloft and 3 neither, an almost even split. One person can be in several rows:
What people noticed On Lexapro On Zoloft Less anxiety 36 28 Tiredness 31 18 More anxiety 19 31 Emotional flatness 23 16 Stomach upset 5 24 Weight gain 19 18
When people who had taken both explained why they switched, the most common reason was side effects (35 people), then the first one stopped working (28) or never helped (12).
How do doctors choose between Lexapro and Zoloft? Clinical section: medical review pending
The choice usually comes down to your main symptoms, other conditions and how you have responded before. Zoloft's wider list of approved uses makes it a common choice when OCD, PTSD or panic is part of the picture. The clinical view:
In large comparisons most antidepressants work about equally well on average. They differ more in side effects, interactions and how hard they are to stop. Needing a second choice does not mean the first was a mistake.
Prescribers look at your main symptoms, other conditions, other medications and past response, yours or a close relative's. Some antidepressants are more activating and others more sedating, which can be matched to problems with sleep or energy.
Moving from one to the other is planned with your prescriber, since stopping the first too fast can cause withdrawal. Our guide to switching antidepressants explains the usual approaches.
Is Lexapro or Zoloft better for anxiety?
An approved use tells you which kind of anxiety a drug was tested for, not that the other one won't help yours. Prescribers also use these drugs outside their labels. In our Reddit count, less anxiety was the effect people mentioned most on Lexapro, and among the most common on Zoloft, though more anxiety came up too, more often on Zoloft. To help your prescriber choose, describe whether your anxiety is mostly constant worry, panic attacks, social fear or reactions to a past trauma.
Which is safer in pregnancy, Lexapro or Zoloft?
Neither label calls one safer. Zoloft's label says studies of women who took sertraline in the first trimester suggest no difference in major birth defect risk from the background rate, though some studies reported increases in specific defects, with inconclusive results. Lexapro's label says published data have not established an increased risk of major birth defects with SSRIs, and both labels describe risks with SSRIs late in pregnancy: breathing and feeding problems in some newborns and a rare lung condition called PPHN.
Stopping has its own risk. Lexapro's label notes that women who stop an antidepressant in pregnancy relapse into depression more often than women who continue. Bring to your prescriber how far along you are, what you take now and how it has gone, and how past pregnancies or dose changes went.
What should you ask your prescriber when choosing between them?
A clear picture of your symptoms and history makes the choice faster. If you have tried one already, details of how it went are the most useful thing you can bring. It helps to ask or tell them:
which symptom matters most: low mood, generalized worry, panic, OCD or trauma how you did on either drug before, or on any other SSRI whether your stomach is sensitive, or tiredness is already a problem which side effects you would find hardest to live with whether you're pregnant, planning a pregnancy or breastfeeding if switching, how the change from one to the other will be planned
Some changes can't wait for a planned visit. If your mood drops sharply or thoughts of harming yourself appear, especially in the first weeks or after a switch, contact your prescriber the same day. Both labels carry a boxed warning that this risk is higher in people aged 24 and younger. In the US, call or text 988, or find a crisis line .

Frequently asked questions Can Lexapro or Zoloft be taken by teenagers? Zoloft is approved for OCD from age 6; Lexapro for depression from 12 and generalized anxiety from 7. Both carry a boxed warning that suicidal thoughts can rise in people 24 and younger, so a teenager starting either needs closer follow-up in the first months.
Can you take Lexapro and Zoloft together? No, they are not usually combined. Both raise serotonin, and taking two SSRIs together increases the risk of serotonin syndrome. Moving from one to the other is planned by your prescriber.
Which causes more weight gain, Lexapro or Zoloft? In our Reddit count, weight gain came up about as often on both: 19 people on Lexapro and 18 on Zoloft. Weight change varies a lot between people. Raise a steady change early with your prescriber.
Is it easy to switch from Zoloft to Lexapro? Many people switch between them, most often because of side effects or because the first stopped working. The switch needs a plan from your prescriber to limit withdrawal. Keep notes through the change so you can tell the two apart.
Which is better for OCD, Lexapro or Zoloft? Zoloft is FDA-approved for OCD in adults and children 6 and older; Lexapro is not approved for OCD. Ask your prescriber which fits your symptoms.
