Dose vs occupancy
This taper is spread across 22 gradual steps. The curve below shows why the last few milligrams matter most: at low doses, small dose changes produce large drops in serotonin-transporter occupancy.
What this shows: the non-linear relationship between dose and serotonin transporter (SERT) occupancy. The curve is flat at high doses — most transporters are already blocked, so a dose cut has little biological impact — and much steeper at low doses, where even a 1–2 mg reduction causes a disproportionately large drop in occupancy. Escitalopram is the S-enantiomer of citalopram and is among the more potent SSRIs milligram-for-milligram. Its half-life is about 27–32 hours. What coming off Lexapro actually feels like, week by week
Escitalopram clears over a few days, so withdrawal after an oversized cut usually shows up within two to five days: dizziness, brain zaps, irritability, tearfulness, insomnia or vivid dreams, flu-like aches, and a surge of anxiety that can feel like the original problem returning.
Because Lexapro is potent, many people sail through 20 → 10 mg and then struggle from 10 mg downward. In a well-paced taper each step should cause only a brief wobble that settles within one to two weeks. The final few milligrams often take as long as the entire upper half of the plan.

If it gets worse: pausing and reinstatement
If symptoms after a cut are intolerable, hold longer at the current dose, partially reinstate the last stable dose, or contact your prescriber if mood is deteriorating. Escitalopram's ~30-hour half-life means relief after reinstatement often arrives within one to two days.
Once settled, resume from the tolerated dose with smaller steps and longer holds. Pausing is a normal part of learning your personal taper schedule, not a failure. Do not change your dose without clinical advice.
