Zoloft Dosage: Sertraline 25 mg, 50 mg and 100 mg
Most adults start Zoloft at 50 mg a day for depression, or 25 mg for panic, PTSD and social anxiety, and the dose can go up to 200 mg. Here is what each dose is used for, how increases work and what 378 people on Reddit said about their dose.

Key takeaways The usual Zoloft starting dose is 50 mg a day for depression and OCD in adults, and 25 mg for panic disorder, PTSD and social anxiety. If the response is not enough, the dose can rise in 25 to 50 mg steps no more than once a week, up to 200 mg a day. In 314 people on Reddit who named their dose, 50 mg was the most common, followed by 100 mg and 25 mg. Never change your dose on your own; a jump up or a sudden stop can bring side effects or withdrawal.

Zoloft dosage depends on the condition it treats. Most adults with depression start at 50 mg once a day, and people with panic disorder, PTSD or social anxiety start at 25 mg. The dose can be raised in small steps up to a maximum of 200 mg a day.
What is the usual Zoloft dosage to start?
For adults with depression or OCD, the FDA label recommends starting at 50 mg a day; for panic disorder, PTSD and social anxiety disorder, at 25 mg. The label calls either 25 mg or 50 mg the first therapeutic dose.
Condition Starting dose Maximum Depression 50 mg 200 mg OCD, age 13 and older 50 mg 200 mg OCD, age 6 to 12 25 mg 200 mg Panic disorder, PTSD, social anxiety 25 mg 200 mg PMDD, every day 50 mg 150 mg PMDD, only in the 2 weeks before a period 50 mg 100 mg Mild liver problems half the usual dose half the usual maximum
With mild liver problems, both the starting dose and the range are halved, so the top dose is 100 mg rather than 200 mg; Zoloft is not recommended with moderate or severe liver problems.
How is a Zoloft dose increased?
For depression, OCD, panic disorder, PTSD and social anxiety, if the starting dose does not help enough, the label allows increases of 25 to 50 mg a day, once a week at most, up to 200 mg. PMDD follows its own steps, shown in the table. The one-week gap comes from Zoloft's half-life of about a day: in adults it takes about a week for the level in your blood to settle after each change. In older people clearance is about 40% lower, and the level takes 2 to 3 weeks to settle. Judging whether a dose helps takes longer, several weeks, so the week is the minimum gap between steps, not the time to wait for a result. Higher isn't automatically better: a 2019 analysis of 77 depression trials found that for SSRIs as a group, benefit rose through the lower-middle doses and then levelled off, while dropouts because of side effects kept climbing with dose. It converted every dose to a fluoxetine equivalent, so it describes SSRIs in general rather than a particular sertraline dose. The clinical view on what a sertraline dose means:
Whether a dose is low or high depends on the medication's usual range and on you.
The same dose can be a starting dose for one person and a full dose for another. Age, other conditions, other medications and how your body processes the drug all play a part.
Doses usually start low and rise in steps, to keep early side effects manageable. A dose goes up when there has been time to judge the response and the benefit is not enough.
For many people a moderate dose works as well as a higher one, with fewer side effects. Some conditions, such as obsessive-compulsive disorder, often need doses near the top of the range.
Going above the maximum in the prescribing information is a specialist decision. Changing your dose on your own, or taking extra on a bad day, can bring side effects or withdrawal.
The full effect of an antidepressant takes four to eight weeks, which is why a dose is judged over that kind of time.
What should you watch for after a Zoloft dose increase?
New or worse anxiety, agitation, panic attacks, trouble sleeping or thoughts of self-harm after an increase are a reason to call your prescriber right away. The FDA boxed warning asks for close watching for worsening and suicidal thoughts after dose changes, and the risk is higher in people 24 and younger. What dose 314 people on Reddit said they take
We read 100 recent threads about Zoloft doses on r/zoloft, comments included, and 378 people wrote about their own dose. These are people's own accounts online, and people whose dose went smoothly post less, so treat this as a range of experiences. Of the 378, 314 named their current daily dose, and 50 mg was by far the most common, at 124.
Daily dose Of 314 people 25 mg 51 50 mg 124 75 mg 25 100 mg 70 150 mg 13 200 mg 18
The rest named in-between or rarer doses, such as 12.5 mg or 37.5 mg.
Of the 378, 162 said their dose had been raised at some point. Of those 162, 55 said what happened next: 30 felt better, 12 felt worse at first and then better, 11 felt worse and 2 noticed no difference. The side effect most often linked to an increase was a rise in anxiety, named by 23 of the 162.
Is 50 mg of Zoloft a lot?
No. It is the usual starting dose for depression and OCD and sits at the low end of the label's 50 to 200 mg range. For many people it is also the dose they stay on long term.
Is 100 mg of Zoloft a lot?
No. 100 mg is in the middle of the label's 50 to 200 mg range, and it was the second most common dose on Reddit, named by 70 of 314. People usually reach it in steps from 50 mg, at least a week apart. Some conditions, such as OCD, often need doses near the top of the range, while for many people a moderate dose works as well as a higher one.
Is 25 mg of Zoloft effective?
It can be. The label counts 25 mg as a therapeutic dose, not just a test dose, and it is where the label starts the anxiety conditions. For depression the label starts at 50 mg, so 25 mg there is lower than usual. Whether it is enough for you is judged over several weeks with your prescriber.
What should you ask your prescriber about your Zoloft dose?
Dose decisions are easier with a clear account of how the current dose is going. Note how you feel on the dose you take now and for how long, before asking about a change. It helps to ask or tell them:
how many weeks you have been on the current dose which symptoms have improved and which have not which side effects you have, and whether they started after an increase whether a lower start or smaller steps would suit you if you are sensitive to side effects what the plan is if the next dose doesn't help
When the time comes to stop, the dose is lowered gradually rather than stopped at once. What that feels like and how to tell it from relapse is covered on the page on Zoloft withdrawal symptoms .
Download on the App Store After a dose change, side effects and benefits shift over several weeks. The withClaro app lets you log mood, energy, sleep and side effects each day, linked to your dose, and turns it into a report for your doctor.
Frequently asked questions What if a higher Zoloft dose doesn't help? Tell your prescriber how many weeks you have been on it and what has and hasn't changed. Going higher isn't the only option; what comes next depends on your symptoms, side effects and history, and it is a conversation to have with them.
When is the best time of day to take 50 mg of sertraline? Zoloft is taken once a day, in the morning or evening. If it makes you sleepy, the evening may suit you; if it disturbs your sleep, the morning. Pick a time and keep it consistent, and check the switch with your prescriber.
What is the maximum dose of Zoloft? The maximum in the FDA label is 200 mg a day for depression, OCD, panic disorder, PTSD and social anxiety, and 150 mg for daily use in PMDD. Going above it is a specialist decision.
What happens if I take 2 Zoloft by mistake? In the UK, the NHS says taking an extra dose of sertraline can be dangerous and to get help from NHS 111. In the US, call Poison Control at 1-800-222-1222. If someone is very drowsy, has a seizure or can't be woken, call emergency services.
Should a Zoloft dose be based on body weight? No. Adult doses in the label are not set by weight. They depend on the condition, how you respond and side effects. Tell your prescriber about other medications and liver problems, which do change the dose.
