Key takeaways Zoloft is FDA-approved for panic disorder, social anxiety disorder, PTSD and OCD, and is often prescribed off label for generalized anxiety. For panic, social anxiety and PTSD the label starts at 25 mg a day, with a usual range of 50 to 200 mg, and the NHS says starting small makes side effects less likely. Of 134 people on r/zoloft who said how their anxiety changed, 78 said it got better and 26 partly better, some only after a rough start. Anxiety can rise in the first weeks; tell your prescriber rather than stopping on your own.

Zoloft (sertraline) is used for anxiety and is FDA-approved for panic disorder, social anxiety disorder, PTSD and OCD. For generalized anxiety it is prescribed off label, and a 2019 review found it effective there too, though its trials were small. The benefit builds over several weeks, and some people feel more anxious before they feel better.

after about 3 weeks I noticed it improved my anxiety, not to where I need to be but significantly enough to notice a difference
Another described a rough start:
I had a few awful days where I thought things were getting worse. It wasn't all the time or every single day, but some days were bad but I could still push through and function. Week 4 onwards has gotten much better for me now.
Does Zoloft help with anxiety? Clinical section: medical review pending
The trials behind the label say yes for several anxiety disorders. In two 10-week panic disorder studies, the drop in full panic attacks was about 2 a week bigger on Zoloft than on placebo; the label doesn't say how many attacks people had to begin with. Separate trials led to its approval for social anxiety disorder and PTSD.
For generalized anxiety, which is not on the US label, a 2019 review in The Lancet pooled 89 trials with 25,441 people. Duloxetine, pregabalin, venlafaxine and escitalopram had the clearest results there. Sertraline also worked and was well tolerated; there were fewer and smaller trials of it, so the authors were less certain, not that it worked less well. The clinical view:
Antidepressants are a first-line treatment for most anxiety disorders, and their benefit for anxiety builds over several weeks.
People with anxiety can be more sensitive to early side effects, including a brief rise in anxiety. That is why treatment often starts at a low dose and goes up slowly.
How long does it take Zoloft to work for anxiety?
Usually several weeks. The panic trials behind the label ran for 10 weeks with the dose raised gradually, and a third ran for 12 weeks at fixed doses. People on Reddit rarely said when their anxiety started to ease, so we can't give a typical time from our count; what they did say is that 18 of the 134 felt worse first and then better.
How the first weeks usually go is covered in how long Zoloft takes to work .
Why does Zoloft start at 25 mg for anxiety?
For panic disorder, social anxiety and PTSD, the label starts adults at 25 mg a day; for depression and OCD the starting dose is 50 mg. If 25 mg isn't enough, the dose can go up, with at least a week between changes; there is no fixed date by which it must rise. The label doesn't give a reason for the low start, but the NHS says a low starting dose reduces the chance of side effects.
Some people feel jittery, restless or more anxious in the first weeks, which can feel like the anxiety itself. In the label's pooled trials, 8% of adults on Zoloft reported agitation, against 5% on placebo. The label's boxed warning adds that suicidal thoughts can rise in people 24 and younger, especially in the first months and after a dose change, so new or worse anxiety at that age is a reason to call rather than wait.
Other early effects are covered in Zoloft side effects .
What dose of Zoloft is used for anxiety?
For panic disorder, social anxiety and PTSD, the label's usual range is 50 to 200 mg a day. If the response isn't enough, increases go in steps of 25 to 50 mg, no more than once a week, up to 200 mg. The dose is found gradually: in the panic trials, people ended up on different doses, and your prescriber sets yours by how your anxiety responds and how you tolerate it.
What should you tell your prescriber about anxiety on Zoloft?
Tell your prescriber how your anxiety has changed since you started, including any rise in the first weeks, and ask what is expected at this point and when you will review it. It helps to tell them:
When you started Zoloft, your dose and any dose change. Whether anxiety rose at first, and whether it has eased since. Which symptoms have changed: worry, panic attacks, physical tension, sleep. Any side effects that make it hard to keep going. Whether you’re in therapy or have used other medications for anxiety.
Some changes can't wait for the next visit. The label asks you to call right away about new or worse anxiety or panic attacks, feeling agitated, restless, angry or irritable, or thoughts of harming yourself. In the US, call or text 988, or find a crisis line near you .
Download on the App Store Anxiety on Zoloft often eases in steps, sometimes after a rough start. The withClaro app lets you log mood, sleep, energy and side effects each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions What if 25 mg of Zoloft isn't helping my anxiety? For panic, social anxiety and PTSD, 25 mg is a starting dose; the label's usual range is 50 to 200 mg. If anxiety hasn't eased after some weeks, tell your prescriber how long you have been on 25 mg and what has changed, so they can judge whether to raise it.
Can Zoloft make anxiety worse at first? Yes, for some people in the first weeks. That is why the dose for anxiety often starts at 25 mg. Tell your prescriber if it is hard to bear.
Is Zoloft or Lexapro better for anxiety? It depends on the type. Lexapro is approved for GAD, Zoloft for panic, social anxiety and PTSD. For GAD, the 2019 review put escitalopram among the drugs with the clearest results and sertraline in the group with smaller trials.
Should I take Zoloft in the morning or at night for anxiety? Either works. Pick the time that suits your sleep and keep it steady. If a morning dose leaves you jittery, ask about an evening one.
