Lexapro Dosage: Is 10 mg a Low Dose, and When Is 20 mg Used?

Key takeaways The FDA label recommends 10 mg of Lexapro once a day for most adults, with 20 mg as the maximum. For most people 10 mg is enough: in the label's fixed-dose trial, 20 mg showed no greater benefit than 10 mg and more people stopped because of side effects. In 100 r/lexapro threads, 5 mg and 10 mg were the doses people named most often. A dose increase often brings a few weeks of extra side effects before any benefit shows.

What doses 291 people on r/lexapro take Is 10 mg of Lexapro a low dose? When is 20 mg of Lexapro used? What happens when you increase your Lexapro dose? What should you tell your prescriber before a dose change?
No, 10 mg is not a low dose of Lexapro (escitalopram). It is the standard recommended dose for adults with depression or generalized anxiety, and 20 mg is the usual maximum. Lower doses, such as 5 mg, are often used to start or for people who are sensitive to side effects. The doses here are from the US FDA label; advice in other countries can differ, so follow your prescriber's plan.
What doses 291 people on r/lexapro take
We read 100 recent threads about Lexapro doses, comments included, and 384 people described their own dose or a dose change. These are people's own accounts, not a clinical study, and people who struggle with a dose are more likely to post, so treat the numbers as a range of experiences.
Of the 291 who named the dose they take now, 5 mg and 10 mg came up most, and the middle dose was 10 mg. Low doses are common among people who have only just started.

Is 10 mg of Lexapro a low dose?
No, it's a full treatment dose, and many people stay on it long term. For older adults and people with liver problems, 10 mg is also the label's recommended dose, not a starting point on the way to 20 mg. Some prescribers start at 5 mg so that early side effects are milder.
Whether 10 mg is enough shows after several weeks at that dose. The signs that it may not be are symptoms that have barely moved, or that improved and then stalled, while side effects have settled. That is the point to bring to your prescriber.
It helps to prepare for your medication appointment with notes on how you have felt.
When is 20 mg of Lexapro used?
Usually when symptoms have barely moved after several weeks on 10 mg, or improved and then stalled, and the side effects have settled. The FDA label allows an increase to 20 mg after at least a week on 10 mg, and 20 mg is the maximum recommended dose for adults.
The label is also frank about what 20 mg adds. Its fixed-dose trial showed that both 10 mg and 20 mg worked, but failed to show a greater benefit of 20 mg over 10 mg. In those trials, 10% of people on 20 mg stopped because of side effects, against 4% on 10 mg and 3% on placebo.
Other studies point the same way. A 2006 pooled analysis of the same three fixed-dose trials, using one subscale of a depression rating score, found 10 mg clearly better than placebo in moderate depression and 20 mg in moderate to severe depression. A 2019 analysis of 77 studies with 19,364 people, covering SSRIs including escitalopram, plus venlafaxine and mirtazapine, found the best balance of benefit and side effects in the lower part of the licensed range. It converted all doses to fluoxetine equivalents, so it cannot settle 10 mg against 20 mg of escitalopram on its own. For anxiety, a 2019 analysis of 57 trials found higher SSRI doses brought somewhat more benefit, along with more dropouts from side effects. So a common path is to start lower and go up in steps if needed, rather than jumping straight to 20 mg.
What happens when you increase your Lexapro dose?
Side effects often return for a while after an increase. Nausea, trouble sleeping and a short rise in anxiety are common in the first weeks, much like when you started. The new dose acts from the first day; a noticeable difference in how you feel usually takes several weeks.
Of the 384 people on r/lexapro, 111 said their dose had been raised at some point. Of the 54 among them who said what happened next, 29 said it helped, 13 that it felt worse at first and then better, 11 that it felt worse, and 1 noticed no difference. Of all 111 whose dose went up, 14 said their anxiety rose afterwards, some of them only for a while. One person wrote:
my anxiety heightened ALOT for the first 3 weeks from upping to 20 but once it kicked in i boy I felt it.
Another had a smoother time:
15mg was when I finally started feeling better
Watch your mood as well. The FDA label warns that suicidal thoughts can rise in the first months and after a dose change, most of all in people aged 24 and younger. If they appear, get help right away: call your prescriber, or in the US call or text 988, or find a crisis line near you . The clinical view:
Whether a dose is low or high depends on the medication's usual range and on you.
The same dose can be a starting dose for one person and a full dose for another. Age, other conditions, other medications and how your body processes the drug all play a part.
Doses usually start low and rise in steps, to keep early side effects manageable. A dose goes up when there has been time to judge the response and the benefit is not enough.
Going above the maximum in the prescribing information is a specialist decision. Changing your dose on your own, or taking extra on a bad day, can bring side effects or withdrawal.
The wait after a dose change is much like the start, described in how long Lexapro takes to work .
What should you tell your prescriber before a dose change?
Tell your prescriber how you have felt on your current dose over the past few weeks, not just today. That helps them judge whether to wait, raise the dose or try something else. It helps to tell them:
How long you have been on your current dose. What has improved, even slightly, and what has not. Which side effects you have, and whether they're easing. How you felt after any earlier dose change. Any other medications you take, including over-the-counter ones.
Do not change your dose on your own. If thoughts of suicide or self-harm appear after a dose change, call your prescriber the same day. In the US, call or text 988, or find a crisis line near you .
Download on the App Store A dose change on Lexapro can take weeks to settle, and the details blur by the next appointment. The withClaro app lets you log mood, energy, sleep and side effects each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions Is 5 mg of Lexapro enough? For some people, yes, especially those sensitive to side effects or with milder symptoms. It is often a starting dose. Your prescriber judges whether it is enough by how you respond after several weeks.
Is 20 mg of Lexapro a high dose? It is the maximum recommended dose for adults on the FDA label, and it may be the right dose for more severe depression. Before an increase, ask your prescriber what you are hoping it will change and how you will both judge it.
How long does it take to adjust to a Lexapro dose increase? Many people notice side effects for one to a few weeks after an increase, and the benefit can take several weeks. Note how you feel each day and share it at your next appointment.
Can you take 30 mg of Lexapro? Doses above 20 mg are above the maximum in the FDA label. Going higher is a specialist decision. Do not raise your dose on your own.
