Key takeaways In SSRI trials, a small improvement shows by the end of week one and keeps building, more slowly, for at least six weeks. In 87 r/lexapro threads, the typical answer from people who named a time was that Lexapro started helping after about three weeks. The first weeks can feel worse before they feel better, with nausea or a rise in anxiety. New thoughts of suicide or self-harm, especially in people 24 and younger, need a same-day call; in the US, call or text 988.

What does it feel like when Lexapro starts working? Does a higher dose of Lexapro work faster? When should you talk to your prescriber if Lexapro isn't working?
Most people feel Lexapro (escitalopram) working after a few weeks. In SSRI trials, the improvement kept building, more slowly, for at least six weeks, so the full effect takes longer than the first signs. The medicine acts from the first dose; the weeks are the brain's response catching up, which is why side effects often come first. Small changes can show in the first week or two, often in sleep, energy or anxiety, before mood clearly lifts.
When people on r/lexapro said Lexapro started working: about 3 weeks
Of 74 people on r/lexapro who said when they first noticed it helping, the typical answer was about 3 weeks. Some of them were only partly better; among the 59 who said it was working by the time they posted, half named between 2 and 7 weeks. People tend to post when they are struggling or unsure, so these numbers show the range of experiences, not how likely Lexapro is to work for you.
We read 87 recent threads about the early weeks on r/lexapro, comments included, and 279 people described their own course. Of them, 242 said whether Lexapro was helping yet. Most were still early on, so many were waiting rather than done:
Where people were Of 242 people It was working 101 It was partly working 64 Not yet, still waiting 70 It didn't help 7
One person on r/lexapro described a fairly typical course:
It's been 3 weeks and I no longer have panic attacks and anxiety decreased
For others it took longer. Another person wrote:
I really didn't get the full effect until week 10.
The early weeks were often rough. Of the 279 people, 40 said their anxiety got worse at first, and 31 had nausea or stomach trouble.
How long does Lexapro take to work, according to studies?
Studies point to a gradual effect that starts early. A 2006 analysis of 28 trials with 5,872 adults with depression looked at SSRIs as a group, the class Lexapro belongs to, without separating the individual drugs. People on SSRIs improved more than people on placebo by the end of the first week, and the improvement kept going at a slowing pace for at least six weeks. For escitalopram itself, a 2006 pooled analysis of the manufacturer's trials compared it with other antidepressants, not with placebo. By day 7, scores on a depression scale had dropped by 3.9 points on escitalopram against 3.4 on the others. That is half a point: statistically different, but too small to say it starts working noticeably faster.
The NHS says it can take several weeks until symptoms start to improve on escitalopram. That fits what most people describe. Early signs are small, and the change is easier to see over a month than from one day to the next.
How long does it take for Lexapro to work for anxiety?
Several weeks, and it keeps improving after that. A 2019 analysis of 57 trials with 16,056 adults with anxiety disorders found that on SSRIs, symptoms improved at a steady pace across the 12 weeks of treatment, with no sudden turning point. The analysis covered anxiety disorders in general, and the effect was largest in social anxiety. It found no clear difference in effect between individual SSRIs, which is why the result applies to Lexapro too. Lexapro itself is approved by the FDA for generalized anxiety disorder in adults.
That means week two is not the verdict. If anxiety is easing slowly, give it time and keep your prescriber updated.
What does it feel like when Lexapro starts working? Clinical section: medical review pending
Side effects usually arrive first: in the FDA trials for depression, 15% of adults on Lexapro had nausea, against 7% on placebo. The benefit comes after them. The clinical view:
Sleep, appetite or energy may improve first. The lift in mood usually comes later, and the full effect later still. This is why the first weeks can feel like a step backward.
Early signs of progress are often small, and other people may notice them first. Getting out of bed gets easier, you spend less time on the same worries, and old interests come back.
Talk to your prescriber about the dose or a different medication if:
nothing has improved after several weeks at a steady dose side effects are hard to live with
New thoughts of suicide or self-harm in the first weeks need a call to your prescriber the same day.
If those thoughts come and you cannot reach your prescriber, call or text 988 in the US, or find a crisis line . The FDA label warns that this risk is higher in people aged 24 and younger, most of all in the first months and after a dose change. A short rise in anxiety at the start is common and usually settles; if it is hard to manage, ask your prescriber whether it is expected at this stage and what can ease it.
Women can also have side effects men don't, such as changes in the menstrual cycle, covered in Lexapro side effects in women .
Does a higher dose of Lexapro work faster?
Not necessarily. For most adults, 10 mg once a day is the recommended dose for both depression and generalized anxiety, and the label allows 20 mg after at least a week. In fixed-dose depression trials, 20 mg did not show a greater benefit than 10 mg, and more people stopped because of side effects on 20 mg: 10% against 4%. Across SSRIs for anxiety, a 2019 analysis of 57 trials found higher doses brought somewhat more benefit along with more dropouts from side effects.
So a higher dose is a trade-off, not a shortcut. Your prescriber decides on any change, so bring them how you have been feeling over the past few weeks.
The usual range and what changes with each step are covered in Lexapro dosage .
When should you talk to your prescriber if Lexapro isn't working?
Talk to your prescriber if there is still no change after a few weeks. Go sooner if side effects are too much. Notes from those weeks make the conversation more useful than a single bad day. It helps to tell them:
When you started Lexapro, your dose and any dose changes. What, if anything, has improved, even a little: sleep, energy, worry or mood. Which side effects you have had, and whether they're easing. Whether anxiety rose at the start and whether it has settled. Any days you missed a dose.
If you have thoughts of suicide or self-harm, call your prescriber the same day. In the US, call or text 988, or find a crisis line near you .
Download on the App Store Early changes on Lexapro are small and easy to miss from one day to the next. The withClaro app lets you log mood, energy, sleep and side effects each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions Can Lexapro work in 2 days? A few people notice small changes within days, but most need several weeks. A 2006 analysis of SSRI trials found the first measurable gains by the end of week one. Keep taking it as prescribed and note any change, however small.
Why do I feel worse on Lexapro in week 2? Side effects such as nausea and a brief rise in anxiety often come before the benefits, and they usually settle within the first weeks. If they are hard to manage, note when they happen and ask your prescriber whether this is expected at week 2.
How do you know if Lexapro is working? Look for small shifts: getting out of bed is easier, sleep or energy improves, worries take up less time. Other people sometimes notice first. Compare how you feel now with a few weeks ago, then share it with your prescriber.
How long should you give Lexapro before trying something else? Several weeks at a steady dose is the usual test; in our count, half of those who named a time felt it between 2 and 7 weeks. If nothing has changed by then, bring your notes to your prescriber and decide together what comes next.
