Key takeaways Celexa starts at 20 mg once a day and can rise to 40 mg after at least a week. The label doesn't recommend doses above 40 mg because of heart rhythm risk. People over 60, people with liver problems and some others are limited to 20 mg. Of 59 people on Reddit who named their dose, the typical dose was 20 mg.

The usual Celexa dosage is 20 mg of citalopram once a day, with or without food. This page follows the US FDA label; in other countries, such as the UK, the local guidance on limits can differ. Some people start lower, the dose can go up after at least a week, and for people over 60 or with liver problems 20 mg is already the top.

Most adults take 20 mg once a day, and 40 mg is the maximum; for some people the maximum is 20 mg. The label's plan for depression in adults:
Situation Dose Starting dose (label) 20 mg once a day Smaller first dose, if your prescriber chooses (MedlinePlus) from 10 mg; raised at most weekly Maximum (label) 40 mg once a day Over 60, liver problems, CYP2C19 issues (label) 20 mg at most
The label names 20 mg as the starting dose and makes a 10 mg tablet. MedlinePlus says a doctor may begin with a smaller dose and raise it no more often than once a week, so 10 mg is a real starting or ongoing dose for some people. People who process citalopram slowly (CYP2C19 poor metabolizers), or who take cimetidine or another medicine that blocks that enzyme, are also limited to 20 mg. Your pharmacist can check whether any of your medicines do. Many people take citalopram for anxiety too, though the label's plan is written for depression; more in Celexa for anxiety . The clinical view on what a dose means:
Whether a dose is low or high depends on the medication's usual range and on you.
The same dose can be a starting dose for one person and a full dose for another. Age, individual conditions, other medications and how your body processes the drug all play a part.
Doses usually start low and rise in steps, to keep early side effects manageable. A dose goes up when there has been time to judge the response and the benefit is not enough.
Going above the maximum in the prescribing information is a specialist decision. Changing your dose on your own, or taking extra on a bad day, can bring side effects or withdrawal. ## Why is Celexa capped at 40 mg?
The label doesn't recommend more than 40 mg a day because citalopram can lengthen the QT interval, and the effect grows with the dose. QT is the time on an ECG from the start of the heart's main pumping beat to the end of its recovery before the next one; the label refers to it corrected for heart rate, QTc. When it gets too long, the risk of a dangerous irregular heartbeat rises.
The label asks for citalopram to be avoided, unless the benefit outweighs the risk, in people with:
congenital long QT syndrome or a slow heart rate low potassium or magnesium a recent heart attack or heart failure that is not under control
It also warns about combining it with other medicines that lengthen QT, such as some heart rhythm medicines, some antipsychotics, some antibiotics and methadone. Your doctor may order an ECG and blood tests before and during treatment, MedlinePlus says. Fainting, severe dizziness, chest pain or a pounding, irregular heartbeat on citalopram needs medical help the same day.
After any dose change, watch your mood as well as side effects. The FDA label warns that suicidal thoughts can rise in the first months and after a dose change in people aged 24 and younger. If they appear, call your prescriber the same day. If you're having thoughts of harming yourself, contact emergency services or a crisis line straight away. In the US, call or text 988. In the UK, call Samaritans on 116 123. Elsewhere, find a crisis line near you .

What dose 59 people on Reddit said they take
We read 97 recent threads where people discussed citalopram doses, comments included, and 93 people wrote about their own dose. These are people's own accounts, and people who notice a big change are more likely to post, so treat this as a range of experiences. Of the 93, 59 named a number, and the typical dose was 20 mg.
Of the 93, 30 said their dose had been raised, and 13 of them said what happened next: 2 felt better, 2 felt worse at first and then better, and 9 felt worse. Of the 30, 6 named more anxiety after the increase.

What should you ask your prescriber about your Celexa dose?
Dose decisions are easier with a clear account of how the current dose is going. It helps to ask or tell them:
how long you have been on the current dose which symptoms have improved and which have not any heart rhythm problems or palpitations other medicines, especially those that affect heart rhythm what the plan is if the next dose doesn't help
When the time comes to stop, the label asks for a gradual reduction. More in Celexa withdrawal .
Download on the App Store A change in Celexa dose can change sleep, energy and anxiety within days. The withClaro app lets you log mood, sleep, energy and side effects each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions Can you take more than 40 mg of Celexa? The label doesn't recommend it, because the heart rhythm risk grows with the dose. If 40 mg isn't enough, ask your prescriber what else could help rather than going higher.
Is 10 mg of Celexa enough? For some people, yes. The label's starting dose is 20 mg, but MedlinePlus says a doctor may begin with a smaller dose, and some people do well on 10 mg. Whether it is enough for you depends on how you respond after a few weeks, which is a call to make with your prescriber.
Should Celexa be taken in the morning or at night? The label says once a day with or without food, at any time. Pick a time and keep it. If you want to change it, for example because of sleepiness or trouble sleeping, agree it with your prescriber.
My dose went up and nothing changed. What now? Give it a few weeks at the new dose before judging, and keep a note of what has and hasn't changed. Then bring that to your prescriber; at 40 mg, going higher isn't an option, so the next step is a different plan rather than more of the same.
