Key takeaways For depression, the label's range is 40 to 60 mg a day, with 60 mg the target for most; some people start at 30 mg for a week. The label sets 120 mg a day as the maximum, but found no extra benefit above 60 mg for depression or anxiety. Of 218 people on r/cymbalta who named their dose, the middle dose was 60 mg, the label's target. Capsules are swallowed whole, not opened or chewed.

What is the usual Cymbalta dose? How is the Cymbalta dose raised? Is a higher Cymbalta dose better? How should you take Cymbalta? What should you ask your prescriber about your Cymbalta dose?
The usual Cymbalta dosage for depression is 40 to 60 mg of duloxetine a day, and 60 mg once a day is the target for most people. The doses here are from the US FDA label, which allows some people to start at 30 mg for a week so the body can adjust. The ceiling is 120 mg for depression and anxiety and 60 mg for pain conditions.
What is the usual Cymbalta dose?
For depression, the label gives 40 mg a day, as 20 mg twice, up to 60 mg a day, once or as 30 mg twice. For generalized anxiety, adults under 65 start at 60 mg, some after a week at 30 mg; people 65 and older start at 30 mg for 2 weeks before the target of 60 mg is considered. Capsules come in 20 mg, 30 mg and 60 mg.
Condition Usual dose in the label Maximum Depression 40 to 60 mg a day 120 mg Generalized anxiety 60 mg once a day 120 mg Nerve pain from diabetes 60 mg once a day 60 mg Fibromyalgia 30 mg for a week, then 60 mg 60 mg Chronic muscle and joint pain 30 mg for a week, then 60 mg 60 mg
The 120 mg maximum is not a target: the label found no added benefit above 60 mg for depression or anxiety. With chronic liver disease, severe kidney disease or heavy drinking, the label says to avoid Cymbalta altogether.
How is the Cymbalta dose raised?
For depression and anxiety, the usual step is from 30 mg to the 60 mg target. For generalized anxiety, if the prescriber decides to go higher than 60 mg, the label raises it in 30 mg steps, up to 120 mg; for depression it gives 120 mg as the maximum without a step plan. For nerve pain, fibromyalgia and chronic muscle and joint pain, the dose stays at 60 mg. A new dose is usually judged after a few weeks at it, as covered in how long Cymbalta takes to work .
Is a higher Cymbalta dose better? Medically reviewed by Dr. Jason Tan on October 27, 2026, this section only.
Not usually. The label says 120 mg a day worked for depression, but there is no evidence that doses greater than 60 mg add benefit. Some side effects grow with the dose, and the safety of doses over 120 mg hasn't been studied. For fibromyalgia, higher doses brought more side effects without more benefit.
Blood pressure is the clearest example. Cymbalta can cause a drop in blood pressure on standing, with dizziness, falls or fainting, most often in the first week and after a dose increase. The label says the risk is greater at doses over 60 mg a day and with blood pressure medicines. Your prescriber may check your blood pressure before you start and from time to time after. The clinical view:
Whether a dose is low or high depends on the medication's usual range and on you.
The same dose can be a starting dose for one person and a full dose for another. Age, individual conditions, other medications and how your body processes the drug all play a part.
Doses usually start low and rise in steps, to keep early side effects manageable. A dose goes up when there has been time to judge the response and the benefit is not enough.
Going above the maximum in the prescribing information is a specialist decision. Changing your dose on your own, or taking extra on a bad day, can bring side effects or withdrawal. ## What dose 218 people on r/cymbalta said they take
We read 100 recent threads about Cymbalta doses on r/cymbalta, comments included, and 312 people wrote about their own dose. These are people's own accounts online, not a measure of how often anything happens, and people without a big change post less. Of the 312, 218 named a number, and the middle dose was 60 mg, the label's target: 83 were on 60 mg, 57 on 30 mg, 25 on 20 mg, 16 on 90 mg, 12 on 120 mg and 11 on 40 mg.
A higher dose was not a sure win. Of the 76 who said their dose had been raised, 32 said what happened next: 13 felt better, 13 felt worse, 4 felt worse at first and then better, and 2 noticed no difference.
One person on r/cymbalta wrote:
30mg did nothing for my generalized anxiety, but 60mg (30mg x2/day) works great!
For another, the increase did not help:
I was upped to 60mg for a month and it made it worse
How should you take Cymbalta?
Swallow the capsule whole, with or without food; do not open, chew or crush it. If you miss a dose, MedlinePlus says to take it when you remember unless the next one is close, and never to double up.
What should you ask your prescriber about your Cymbalta dose?
Dose decisions are easier with a clear account of how the current dose is going. Note how you feel on your dose now, and for how long, before asking about a change. It helps to ask or tell them:
how long you have been on the current dose which symptoms have improved, including pain, and which have not side effects since the last change any liver or kidney problems, and how much alcohol you drink, since the label rules Cymbalta out with chronic liver disease, severe kidney disease or heavy drinking dizziness when you stand up, or any fall or faint what the plan is if the next dose doesn't help
If your mood drops fast after a dose change, or you have thoughts of harming yourself, contact your prescriber the same day rather than waiting. The label warns this risk is highest in the first months and after dose changes, most of all in people aged 24 and younger. If you're having thoughts of harming yourself, contact emergency services or a crisis line straight away. In the US, call or text 988. In the UK, call Samaritans on 116 123. Elsewhere, find a crisis line near you .
When the time comes to stop, the label recommends lowering the dose gradually. More on that in Cymbalta withdrawal .
Download on the App Store A new Cymbalta dose can change sleep, energy and pain within days. The withClaro app lets you log mood, sleep, energy and side effects each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions Can you take Cymbalta twice a day? Yes. For depression, the label allows 20 mg or 30 mg twice a day as well as once a day. If one daily dose upsets your stomach or sleep, ask your prescriber whether splitting it fits you.
Is 30 mg of Cymbalta enough? For some people, yes, but it's mainly a starting dose, and 60 mg is the label's target for most. If you have been on 30 mg for a few weeks, note which symptoms have changed and ask your prescriber whether to stay or raise it.
Is 120 mg of Cymbalta a lot? It is the label's maximum for depression and anxiety, and the label found no added benefit above 60 mg, while some side effects grow with the dose. If you are on 120 mg, ask your prescriber what it is meant to add for you.
What if you can't tolerate 60 mg of Cymbalta? Tell your prescriber which side effects you have and since when. For depression the label also allows 40 mg a day, as 20 mg twice, or 30 mg twice a day; the decision is one to make together rather than cutting the dose on your own.
