Key takeaways Remeron starts at 15 mg once a day, preferably in the evening before sleep. The label allows raising the dose up to 45 mg a day, with at least 1 to 2 weeks between changes. Of 82 people on Reddit who named their dose, the typical one was 15 mg, and 15 took 7.5 mg, a dose not in the label. Older adults and people with kidney or liver problems may need more care or a smaller dose.

The usual Remeron dosage starts at 15 mg of mirtazapine once a day, preferably in the evening before sleep. If 15 mg isn't enough, the FDA label allows raising it up to 45 mg a day. Dose changes should be at least 1 to 2 weeks apart.
What is the usual Remeron dosage?
For adults with depression, the label sets one plan, from 15 mg in the evening up to 45 mg, and the label adds that some people need a smaller or larger dose, or more care, depending on age, other conditions and other medicines.
Who What the label says Adults with depression 15 mg once a day in the evening, up to 45 mg, at least 1 to 2 weeks between changes Older adults use with caution; clearance is lower, and it can cause confusion and over-sedation Moderate to severe kidney or liver problems a smaller dose may be needed Taking carbamazepine, phenytoin or rifampin a larger dose may be needed Taking cimetidine a smaller dose may be needed
Tablets come in 15 mg and 30 mg, both scored, and the SolTab in 15 mg, 30 mg and 45 mg.
7.5 mg is not part of the Remeron label's plan, but some prescribers use it; that is off-label, meaning outside the label's plan. If you have been prescribed it, ask your prescriber why that dose was chosen and how you will judge whether it works.
How should you take Remeron?
Take it once a day at bedtime, with or without food. The SolTab stays in its blister until you are ready: open it with dry hands, put it on your tongue, and let it dissolve; don't split it. If you miss a dose, MedlinePlus says to take it when you remember unless the next one is nearly due, and never to double up.
When is a Remeron dose raised? Medically reviewed by Dr. Jason Tan on October 27, 2026, this section only.
When the current dose has had time to show what it does and the benefit isn't enough. The label sets the gap at no less than 1 to 2 weeks, to allow time to judge the response to each dose. Older adults clear mirtazapine more slowly: in the label's study, clearance was 40% lower in older men and 10% lower in older women than in younger people, so steps are taken with more care. Higher isn't always better: a 2019 analysis of antidepressant doses, which included 11 mirtazapine treatment groups, found that its effect rose up to about 30 mg and then fell, and that fewer people dropped out in the lower part of its range. That is an average across trials, not a rule for you. The clinical view:
Whether a dose is low or high depends on the medication's usual range and on you.
The same dose can be a starting dose for one person and a full dose for another. Age, individual conditions, other medications and how your body processes the drug all play a part.
Doses usually start low and rise in steps, to keep early side effects manageable. A dose goes up when there has been time to judge the response and the benefit is not enough.
Going above the maximum in the prescribing information is a specialist decision. Changing your dose on your own, or taking extra on a bad day, can bring side effects or withdrawal. If your mood drops sharply or thoughts of self-harm appear after a dose change, contact your prescriber right away rather than waiting for the next visit; the label asks that everyone on an antidepressant be watched closely for worsening and for suicidal thoughts. If you're having thoughts of harming yourself, contact emergency services or a crisis line straight away. In the US, call or text 988. In the UK, call Samaritans on 116 123. Elsewhere, find a crisis line near you .
What dose do 82 people on Reddit actually take?
We read 100 recent threads where people discussed mirtazapine doses, comments included, and 102 people wrote about their own dose. Most of the threads were on r/Anxiety, so many people took it for anxiety or sleep, while the label's doses are for depression. People who notice a big change are more likely to post, so treat this as a range of experiences. Of the 102, 82 named their dose in milligrams, and 15 mg was the most common:
Daily dose Of 82 people 7.5 mg 15 15 mg 38 30 mg 14 45 mg 11
The other 4 named rarer doses. Of all 102, 18 said their dose had been raised at some point. Some were wary of going up; one wrote:
I myself was wary of going up on a higher dose, my starting dose was 15mg
What should you ask your prescriber about your Remeron dose?
Dose decisions are easier with a clear account of how the current dose is going, especially with Remeron, where sleepiness, appetite and morning grogginess can change with the dose. Ask what the next step would be and how long to wait before judging it. It helps to ask or tell them:
how long you have been on the current dose how sleepy you feel in the morning changes in appetite or weight which symptoms have improved and which have not what the plan is if the next dose doesn't help
Bring a note of when you take it, how you slept and how you felt the next morning for the week or two before the visit. How sleepiness changes with the dose is covered in whether Remeron makes you tired .
Download on the App Store A new Remeron dose can change sleep, appetite and morning grogginess. The withClaro app lets you log sleep, appetite, energy and mood each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions Is 30 mg of Remeron a high dose? No. It's the middle of the label's range, which runs up to 45 mg, and close to the point where, in a 2019 dose analysis, mirtazapine's effect stopped rising. If you are on 30 mg and it isn't enough, ask your prescriber what the next step would be.
Is 45 mg of Remeron the maximum? Yes, 45 mg once a day is the label's maximum. The label warns that doses above the recommended range can have serious outcomes, including deaths, especially when mixed with other drugs. Never take more than prescribed; if someone has, call Poison Control at 1-800-222-1222 in the US, or 911 if they collapse.
Should I take Remeron at night? The label recommends the evening, before sleep, because it causes sleepiness.
Can I split a Remeron tablet? The 15 mg and 30 mg tablets are scored, but the SolTab should not be split. Ask your pharmacist before splitting any tablet.
