Pill splitting Pill splitting is cutting a tablet into parts to get a dose no tablet comes in, most often during a taper. It suits scored tablets, and others a pharmacist confirms can be split, using a tablet cutter. Extended-release, enteric-coated and orally disintegrating tablets should not be split unless the leaflet says it is safe, and capsules cannot be. Pieces from unscored tablets can be uneven.
Also called: Tablet splitting, Pill cutter.

What pills cannot be split? Extended-release tablets are built to release the drug slowly, and cutting them can release it all at once. Their names often include XL, XR, ER, SR or CR, but not always: Pristiq (desvenlafaxine) has none of these letters, yet it is an extended-release tablet, and its label says to swallow it whole, without dividing, crushing or chewing it. Bupropion SR and XL are not split either. Capsules are not cut; opening one is a separate question, covered under bead counting. The prescribing information for your product says whether its tablets can be divided.
Which antidepressants can be split? Ordinary tablets whose label allows it. Scored tablets, with a line across them, are made to be broken: escitalopram (Lexapro) 10 mg and 20 mg tablets are scored, for example, while the 5 mg tablet is not. The table below shows the smallest US tablet of each medication in the withClaro taper calculator that comes as a tablet, and whether it is scored; your pharmacist can check your exact product.
Smallest US tablet and whether it is scored What is the best way to split a pill? Use a pill cutter, a small blade in a holder that keeps the tablet still. Scored tablets break more evenly. Halves are usually close in size, quarters less so, and small differences matter more at low doses.
What people on Reddit say
More people skipped days on purpose than split tablets. Of 1,185 people who described coming off an SSRI or SNRI on Reddit, 30 said they cut or split tablets to lower their dose, and 69 said they took a dose every other day or skipped days on purpose. People who write about coming off are not typical of everyone who tapers, so these counts show what happens, not how often.

What if half a tablet is still too much? Then splitting has reached its limit for that step. Ask your prescriber and pharmacist what comes next: some medications already have a licensed liquid, such as escitalopram oral solution, and a compounding pharmacy can make other doses on prescription.
Do pharmacies give pill cutters? Most pharmacies sell them rather than give them away, and some will split tablets for you if you ask.
What to do Ask your pharmacist whether your tablet can be split and whether a lower strength exists before you cut it; a lower-strength tablet is often more accurate than a cut one.
