Liquid antidepressants A liquid formulation is a medicine made as a liquid instead of a tablet: an oral solution, ready to measure, or a concentrate, a stronger liquid that is diluted just before you take it. With antidepressants it lets you measure doses smaller than any tablet, which helps in the last steps of a taper. Not every antidepressant comes as a liquid, and a liquid dose is not always the same as the tablet dose in milligrams, so the change is worked out with a prescriber or pharmacist and measured with an oral syringe.
Also called: Liquid formulation, Oral solution, Liquid Lexapro, Liquid sertraline, Oral concentrate.

Is there liquid Lexapro or liquid sertraline? Yes, both. In the US, escitalopram (Lexapro) comes as an oral solution, and sertraline (Zoloft) as a liquid that is diluted before use, by their prescribing information. Three other SSRIs, paroxetine, citalopram and fluoxetine, have US oral liquids too, and so does nortriptyline, an older tricyclic antidepressant. In the UK and other countries the list differs, so ask your pharmacist. For other antidepressants, a compounding pharmacy can make a liquid on prescription. The full list for the US is in the table below.
Which antidepressants come as a liquid in the US? Why use a liquid to taper? The final steps of a taper often need doses smaller than the smallest tablet. UK guidance from NICE says that once very small doses are reached and the taper cannot go on with tablets or capsules, liquid preparations can be used where available.
What people on Reddit say
Of 1,185 people who described coming off an SSRI or SNRI on Reddit, 18 measured their lower doses as a liquid, fewer than those who split tablets (30) or counted capsule beads (83). Most liquid users, 13, were on r/lexapro, which has a liquid form. These are self-reports of people who chose to write about it, so they show how it happens, not how often.

How is a liquid dose measured? With an oral syringe marked in milliliters, which the pharmacy can provide. The label tells you how many milligrams are in each milliliter, and that number differs between products. For example, Zoloft’s liquid is 20 mg per mL, so a 25 mg dose is 1.25 mL; Lexapro’s liquid is 1 mg per mL, so check the strength on your own label. Zoloft’s liquid comes with a dropper marked only at 25 mg and 50 mg, and its label says to mix each dose with half a cup of water, ginger ale, lemon or lime soda, lemonade or orange juice, nothing else, and take it straight away. For doses between the dropper marks, the Royal College of Psychiatrists describes using an oral syringe instead. Ask the pharmacist to show you how to draw up your dose before your first one.
Is a liquid the same medicine as the tablet? It contains the same active drug, so the same dose in milligrams is meant to work the same way as the tablet. Some people still notice a change after switching forms, so watch how you feel in the first days. If you move from tablets to a liquid, or from one liquid to another, check the new dose in milligrams with your pharmacist so it matches the plan, since a commercial liquid and a compounded one can differ in strength.
What to do Ask your prescriber or pharmacist whether your medication comes as a liquid where you live, or whether a compounding pharmacy can make one. Bring the dose you are aiming for, so they can tell you which strength fits.
