Direct switch between antidepressants A direct switch means stopping one antidepressant and starting another the next day, with no overlap in the medication you take and no gap between them. Prescribers use it mainly between two medications of the same kind, such as two SSRIs. It is not used when switching from fluoxetine, which stays in the body for weeks, or to or from an MAOI.

Can I switch directly from one SSRI to another? Sometimes. A direct switch is most often used between medications that work in a similar way, such as two SSRIs, when the dose of the first is not high. Because both block the reuptake of serotonin, the second takes over part of that effect from the next day, which can soften withdrawal from the first.
People usually switch because the first medication did not help enough or its side effects were hard to live with, a 2016 review in Australian Prescriber notes. Fluoxetine is an exception: it stays in the body for weeks after the last tablet, so a switch away from it is planned differently, often with a gap of several days before the new medication starts at a low dose. Your prescriber decides based on the two drugs and your dose. Doses do not convert exactly from one antidepressant to another.
When is a direct switch not used? When the two medications cannot be taken close together, as when switching to or from an MAOI, an older group of antidepressants. Taking an SSRI and an MAOI close together can cause serotonin syndrome, so a washout period, a gap without either medication, is needed instead. The Lexapro prescribing information, for example, requires at least 14 days between stopping an MAOI and starting Lexapro, and the same after stopping Lexapro before an MAOI. After fluoxetine, the gap before an MAOI is at least 5 weeks, because it stays in the body so long. Before another SSRI the gap is usually much shorter. The table below gives the half-lives of five common antidepressants, fluoxetine among them.
Half-life by antidepressant Half-lives of five common antidepressants, shortest first.

What does the first week after a direct switch feel like? Two things can happen at once. Withdrawal from the first medication can bring dizziness, nausea, brain zaps, poor sleep or irritability. Early side effects of the second often include nausea, trouble sleeping and feeling on edge, by the prescribing information of common SSRIs. Both tend to show up in the first days, which makes it hard to tell which is which. You may not feel the effect of the new medication for a few weeks, so it is too early to judge it in the first days.
Is a direct switch easier than a cross-taper? Usually simpler and shorter, but not always gentler: you take two medications for fewer days, and the first one stops at once. In a cross-taper, the first medication comes down while the second goes up, so you take both for days or weeks. The trade-off of a direct switch is that the first medication stops at once, so withdrawal from it is more likely if the second does not cover the drop. Which one suits you depends on the pair of drugs and your dose.
What people on Reddit say
Few of the people who said how they switched went straight from one medication to the other. Of 1,185 people who described coming off an SSRI or SNRI on Reddit, 81 said they switched to another antidepressant. Of the 33 who said how, 25 overlapped the two medications in a cross-taper; only a few went straight from one to the other (4) or left a gap (4). People who write about switching may not be typical, so this shows how it happens, not how often.

What to do Ask your prescriber why a direct switch suits your two medications, what to expect in the first two weeks, and when to call them. Write down any new symptom with the day it started. If your mood drops sharply or you feel very agitated, contact them now rather than at your next appointment; for thoughts of harming yourself, call or text 988 in the US or your local emergency number.
