Prozac bridge A Prozac bridge is an informal name for a way of coming off a short-acting antidepressant by switching to fluoxetine (Prozac) and then tapering the fluoxetine instead. Fluoxetine and the substance it turns into leave the body over weeks, so after each cut the level falls slowly on its own instead of dropping within a day. It is off label and based on clinical experience rather than trials. The switch needs a prescriber, because fluoxetine works differently from some of the drugs it replaces and can interact with them.
Also called: Fluoxetine bridge, Bridging with Prozac.

How does a Prozac bridge work? By its US prescribing information, fluoxetine has a half-life of 4 to 6 days with regular use, and norfluoxetine, which it turns into in the body, lasts 4 to 16 days. Switching to it from a medication that leaves the body fast can make the remaining steps gentler. A 2015 review of withdrawal management in Therapeutic Advances in Psychopharmacology says switching to fluoxetine before stopping may be advisable, while noting that there are no controlled trials and that the guidance that exists is conflicting.
Who uses a Prozac bridge? People who have had a hard time coming off venlafaxine, paroxetine, duloxetine or desvenlafaxine, which all leave the body quickly, are the ones this is usually tried for, often after earlier attempts to stop brought on withdrawal. The table below compares venlafaxine and paroxetine with fluoxetine.
What people on Reddit say
Most of the people who started fluoxetine to come off another antidepressant were on the venlafaxine forum. Of 1,185 people who described coming off an SSRI or SNRI on Reddit, 15 said they started fluoxetine to help them come off another antidepressant, 10 of them on r/effexor. People who write about coming off are not typical of everyone who stops, so this shows how it happens, not how often.

Does a Prozac bridge always work? No. The evidence comes mostly from clinical experience, with few trials, and some people still get withdrawal from the first medication during the switch. Fluoxetine also has to be tapered in the end, and it can cause side effects of its own. It also slows a liver enzyme, CYP2D6, that breaks down paroxetine and venlafaxine, so while the two overlap their levels can rise; that is one more reason the switch is planned by your prescriber. Withdrawal symptoms that keep going or get worse in the weeks after the switch are a sign to tell them, rather than waiting for the next appointment.
Is a Prozac bridge the same as a cross-taper? No. A Prozac bridge always switches to fluoxetine; a cross-taper can switch to any antidepressant. In a cross-taper, the first medication comes down while the second goes up, so for a while you take both. A Prozac bridge is often done that way, or as a direct switch. The difference is the goal: in a Prozac bridge, fluoxetine is only a stop on the way to stopping altogether, so its own taper is planned from the start.
What to do If coming off a short-acting antidepressant has been hard, ask your prescriber whether a switch to fluoxetine makes sense for you, how the switch would be done, and how the fluoxetine would be tapered afterward. If you feel much worse or have thoughts of harming yourself, contact your prescriber now rather than at your next appointment, and for thoughts of self-harm call or text 988 in the US or your local emergency number.
