More about Effexor
Key takeaways
- Effexor XR is extended-release venlafaxine, approved in the US for depression, generalized anxiety, social anxiety and panic disorder.
- Effexor is an SNRI: at low doses it acts mainly on serotonin, and norepinephrine joins in as the dose rises.
- The usual starting dose is 75 mg once a day with food; the maximum is 225 mg for depression, generalized anxiety and panic disorder, and 75 mg for social anxiety.
- It can raise blood pressure, and stopping it needs a slow, planned taper.
Effexor XR is used for depression and for three anxiety disorders: generalized anxiety, social anxiety and panic disorder. Its generic name is venlafaxine. It comes in two forms: the once-daily extended-release XR, which this page is about, and immediate-release tablets taken two or three times a day, now sold only as generics. It is one of the most prescribed antidepressants in the US, about 16.3 million prescriptions in 2024 by ClinCalc estimates.
What is Effexor used for?
Depression, generalized anxiety, social anxiety and panic disorder, all in adults. They are dosed differently, so the condition you were prescribed it for matters; if it isn't clear which yours is for, ask, since the dose ceiling depends on it. MedlinePlus adds that it is sometimes used off-label for hot flashes after menopause or during breast cancer treatment.
| Condition | Starting dose | Maximum |
|---|
| Depression | 75 mg, or 37.5 mg for 4 to 7 days first | 225 mg |
| Generalized anxiety | 75 mg, or 37.5 mg for 4 to 7 days first | 225 mg |
| Social anxiety | 75 mg | 75 mg; higher doses showed no added benefit |
| Panic disorder | 37.5 mg for 7 days | 225 mg |
Is Effexor an SSRI?
No. Effexor is an SNRI, a serotonin and norepinephrine reuptake inhibitor. SSRIs such as Zoloft and Lexapro act mainly on serotonin. Cymbalta is another SNRI. For the other types, see the list of antidepressants.
How does Effexor work?
Medically reviewed by Dr. Jason Tan on October 27, 2026, this section only.
After a nerve cell releases serotonin or norepinephrine, the cell takes it back up; venlafaxine slows that reuptake, so the messengers stay around longer. The label says the exact way this treats depression is unclear. The dose shapes the effect: venlafaxine binds the serotonin transporter about 30 times more strongly than the norepinephrine one, so at low doses it acts mostly like an SSRI, and its effect on norepinephrine builds as the dose goes up. The clinical view:
Antidepressants change how the brain uses chemical messengers, but how this lifts mood is not fully understood.
The chemical change starts within hours, yet the full effect on mood takes weeks. Current thinking points to early changes in how the brain processes emotion, then slower changes in receptors, in brain circuits that handle emotion, and in how the brain forms new connections.
The idea that depression comes from a shortage of serotonin is now considered too simple. That leaves the mechanism unclear, as with many effective medicines. Trials show antidepressants help more than placebo, with the clearest benefit in moderate to severe depression. ## What doses does Effexor come in?
Effexor XR capsules come in 37.5 mg, 75 mg and 150 mg, taken once a day with food. Swallow capsules whole without dividing, crushing or chewing them. More in Effexor dosage.
What side effects does Effexor cause?
In the trials, the side effects seen at least twice as often as on placebo were nausea (30%), sleepiness (15%), dry mouth (15%), sweating (11%), loss of appetite (10%), constipation (9%) and sexual side effects, including ejaculation problems in 10% of men and lower sex drive in 5%.
Like every US antidepressant, it carries an FDA boxed warning: the risk of suicidal thoughts and behavior is higher in children, teenagers and young adults 24 and younger, especially in the first months and after a dose change, and Effexor XR is not approved for children. Does Effexor raise blood pressure?
It can, and the rise is dose-related, which sets it apart from most SSRIs. Across its trials, a sustained rise in diastolic pressure of 15 points or more, to 105 or higher, happened in 1.4% of people on Effexor XR against 0.9% on placebo. The label asks for high blood pressure to be controlled before starting and for it to be checked regularly during treatment.
Why does stopping Effexor need a taper?
Venlafaxine has a half-life of about 5 hours, and its active form about 11 hours, so levels fall quickly. The label says withdrawal symptoms are more likely at higher doses and after longer use, and asks for a gradual reduction. More in Effexor withdrawal.
What should you tell your prescriber before starting Effexor?
Some conditions and medications change whether Effexor is a good fit, so name them at the first appointment. One combination is ruled out: an MAOI antidepressant, which must be stopped at least 14 days before starting Effexor, and Effexor stopped at least 7 days before starting an MAOI. Before you start, tell your prescriber about:
- high blood pressure or heart problems
- kidney or liver problems
- any MAOI, other antidepressants or migraine medicines, which raise serotonin syndrome risk
- aspirin, anti-inflammatory pain relievers such as ibuprofen, or blood thinners such as warfarin, which add to bleeding risk
- glaucoma, seizures or a history of mania
- pregnancy, plans for pregnancy or breastfeeding
Download on the App Store Effexor can change sleep, energy and mood on different timelines. The withClaro app lets you log mood, sleep, energy and side effects each day, linked to your dose, and turns it into a report for your prescriber.
