Key takeaways Pristiq (desvenlafaxine) is FDA-approved for depression in adults, so using it for anxiety is off label. It is closely related to Effexor, which is approved for generalized anxiety, social anxiety and panic disorder. In nine trials in adults with depression, anxiety symptoms eased more on desvenlafaxine than on placebo. This held at the usual 50 mg dose too. In 100 r/Pristiq threads, 83 of the 160 people who described their anxiety said it got better, and 35 partly better. People in a hard patch are more likely to post. You may feel the effect on anxiety after a few weeks, and the start can be rougher.

On r/Pristiq they ask practical things. Will it calm the anxiety, how rough is the start, and what should they ask first?

People in a hard patch are more likely to post, so treat these numbers as a range of experiences. They do not give your odds.
Is Pristiq approved for anxiety?
No. The FDA approves a medication for the conditions it was tested for in its own trials. Pristiq was tested and approved for major depressive disorder in adults. Effexor XR went through trials in anxiety disorders and is approved for generalized anxiety, social anxiety and panic disorder.
Desvenlafaxine is the main active form that the body makes from venlafaxine. Sharing an active form does not transfer an approval, so using Pristiq this way stays off label. Prescribers still use it because it acts the same way, so ask yours why it fits your situation.

Does Pristiq work for anxiety?
The most direct evidence comes from people with depression. A 2010 analysis pooled nine eight-week placebo-controlled trials of desvenlafaxine, run by its maker, in about 2,900 adults with major depressive disorder. Anxiety symptoms eased more on desvenlafaxine than on placebo, including at the usual 50 mg dose.
Those people had anxiety alongside depression, not an anxiety disorder as their main diagnosis. For anxiety disorders, the evidence is for the drug classes as a whole. A 2019 analysis of 57 trials with 16,056 adults found SSRIs and SNRIs both effective. It saw no clear difference between drugs within each class. Its summary does not report desvenlafaxine separately, so this is a class result, not Pristiq data.
In the 2019 analysis, the biggest gains with SNRIs tended to come early in treatment. Higher SNRI doses did not bring more benefit. Higher doses of both classes made people more likely to stop because of side effects. The Pristiq label points the same way: 50 mg is the recommended dose, and higher doses showed no added benefit.
Antidepressants are a first-line medication for most anxiety disorders, alongside talking therapy, and their benefit for anxiety builds over several weeks.
They are used for generalized anxiety, panic disorder and social anxiety, often alongside therapy. Some are approved for anxiety, and others are prescribed off label. Off label means the medication is used for something it is not formally approved for. Prescribers do this when there is good evidence and experience behind the use.
People with anxiety can be more sensitive to early side effects, including a brief rise in anxiety. That is why treatment often starts at a low dose and goes up slowly.
The right choice depends on the type of anxiety and any depression alongside it. Past response and the side effects that matter most to you count too. Fast-acting calming medications such as benzodiazepines (for example lorazepam or alprazolam) work sooner but can cause dependence and withdrawal, so they are usually kept short-term.

The Pristiq label treats 50 mg as both the starting dose and the usual treatment dose. Before you start, agree with your prescriber how long to give it. Ask too whether to begin lower and what to do if the start is rough.
What are the side effects of Pristiq?
If you take Pristiq for anxiety, the side effects that matter most are the ones that feel like anxiety itself. In the FDA trials of the 50 mg dose, anxiety as a side effect was uncommon: 3% against 2% on placebo. Trouble sleeping affected 9% against 6%. In our count of 100 r/Pristiq threads, 10 of the 160 people who described their anxiety said it got worse. Another 8 said it was worse at first, then better.
Nausea is the most common side effect overall. In those trials, 22% of people on 50 mg had it, against 10% on placebo. Dizziness affected 13% against 5%, and sweating 10% against 4%.
Pristiq can raise blood pressure, so the label advises regular checks. Tell your prescriber about any heart or blood pressure problems before starting, and about anxiety that gets worse rather than settling.
What should you ask your prescriber about Pristiq for anxiety?
Ask why Pristiq suits your anxiety, what to expect early on and how you will judge whether it works. Clear answers make the early weeks easier to sit through. It helps to ask:
Why Pristiq rather than an option approved for my type of anxiety? What should I do if anxiety rises in the first weeks? How long should I give it before we decide whether it works? Do I need blood pressure checks, and how often? What happens if I want to stop later on?

Tracking your anxiety on Pristiq
Notes on when anxiety rose or eased, next to your dose date, show your prescriber a trend, not a single bad day. Add what else changed that day, such as sleep, a missed dose or a stressful event.
Download on the App Store Anxiety on a new medication rarely improves in a straight line, so good and bad days are hard to weigh. The withClaro app lets you log mood, energy, sleep and side effects each day, linked to your dose, and turns it into a report for your prescriber.
Frequently asked questions How long does Pristiq take to work for anxiety? You may feel the effect after a few weeks, and the pace differs from person to person. The Pristiq label gives no timeline for anxiety, since it is approved for depression. Track how you feel each week and share it with your prescriber.
Can Pristiq help with panic attacks? Some people take it for panic, though direct trials of Pristiq in panic disorder are few. In 100 r/Pristiq threads, 55 of the 231 people who said why they took it named panic attacks or panic disorder. If panic is your main concern, ask your prescriber what has the strongest evidence for it.
Can Pristiq make anxiety worse at first? It can, in the first weeks. Write down when it peaks and how long it lasts. Tell your prescriber if it is hard to manage.
Is it hard to stop Pristiq? It can be. The FDA label advises a gradual reduction, because stopping suddenly can bring withdrawal symptoms such as dizziness and nausea. Plan any stop with your prescriber.
